Outcome metrics

We measure outcomes over outputs using the outcome tool to track individual progress.

A bespoke all-encompassing technological system ensures appropriate care planning and co—ordination of care. This system allows for data collection which focusses on outcomes over output using the Research in Practice for Adults Outcome wheel, which has been adapted to The 7 keys to citizenship.

We use a self assessment tool which is aimed at improving wellbeing in line with Chapter 1 of the Care Act 2014.
